1. EMPLOYEE receives an invitation into E-QIP by Human Resources to complete the SF-85P "Questionnaire for Public Trust Positions". Once the employee completes the investigation in E-QIP, they must print and sign the three signature pages and release the information.
2. EMPLOYEE has two sets of fingerprints taken on the SF-87 OPM fingerprint card ("USOPMOOOZ - FIPC BOYERS,PA" is preprinted in the ORI block). Employees in the Denver/Boulder, CO area should get their fingerprints taken in the Mountain Region Security Office, Rm. 1-4512.
3. EMPLOYEE completes and signs the "Fair Credit Reporting Act of 1970, as amended" form.
4. EMPLOYEE completes an OF-612 "Optional Application for Federal Employment" OR a Resume and an OF-306 "Declaration for Federal Employment".
5. REQUESTING OFFICE submits all completed forms to the Mountain Region Security Office with a project/task number for billing purposes. The cost for the National Agency Check and Inquiry is $100.

7. REQUESTING OFFICE submits all forms with a billing memo to the Mountain Region Security Office giving the organization and task number for billing the ANACI Investigation. Crosby, Director, Office of Executive Support, giving the reason (unclassified) for subject's access to "Sensitive Compartmented Information" (SCI), and the specific compartment types (TK, SI, Gamma, etc.) to which he/she needs access.
7. AUTHORIZING OFFICIAL completes and signs a billing memo to the Mountain Region Security Office giving the organization and task number, stating that they may be billed by the Office of Personnel Management for this Single Scope Background Investigation ($3,900).8. 1. EMPLOYEE completes "U.S. Department of Energy, Security Acknowledgment" DOE F 5631.18.
2. EMPLOYEE has one set of fingerprints (SF-87s) taken with the
OPM ORI number "USOPMOOZ".
3. REQUESTING OFFICE submits a thorough justification letter of the employee's requirement for the "Q" clearance. The memo should be addressed to Robert Hubbard, Acting Chief, Personnel Security Branch, DOE through Wendy Monroe, Mountain Region Security Office.
4. REQUESTING OFFICE submits a billing memo to the Mountain Region Security Office giving the organization and task number for billing the Single Scope Background Investigation ($3900).

INSTRUCTIONS FOR REQUESTING WAIVERS OF
BACKGROUND INVESTIGATIONS
1. Read Chapter 3, 302b. "General Requirements for Applicants and Employees" and Appendix D, Section B.V. of the Personnel Security Manual on Waiver requests. Under the waiver process, agencies are able to hire or promote individuals into critical-sensitive positions prior to completion of the Background Investigation (BI). A waiver request must justify the appointment as an emergency and in the National interest.
2. Address waiver memorandum to Al Broadbent, Director, Office of Security, through Shirley A. Purcell, Personnel Officer, Human Resource Division, from the Operating Unit head.
3. The waiver memo must always state that the appointment is an emergency and in the national interest and must include the position sensitivity designation. Specific examples for waiver justification memos may also include the following:
Promoting the employee as soon as possible is necessary in order that mission-essential duties are performed efficiently.
The subject's new duties are supervisory and necessary for the coordination of the group's work, without which the agency would suffer.
Promoting or hiring the individual is necessary in response to the unit's increased workload, such that the unit or agency would suffer undo hardship without the immediate employment of the individual.4. Any reasonable justification will be considered as long as security considerations do not prohibit approval. ALL waiver requests must be accompanied by a complete BI package, with 35-day expedite authorized.
Waiver approvals take from three to six weeks. The time period for processing ALL investigations is dependent on a variety of issues; i.e. the subject's foreign residence, previous investigation, issues resulting in further processing (financial problems, admitted drug or alcohol use or abuse, involvement with groups whose interests are inimical to the U.S.) and many other considerations.
